Don’t forget your annual sports physical! Below is a link to a generic sports physical form that most Arizona schools use and accept. If your school uses a different form, please try to remember to bring it with you to your appointment. Also, please have your portion of the paperwork filled out prior to your appointment. If you have glasses or contacts, please remember to wear them. Vision testing is required.
